Commercially insured patients may be eligible for GRANIX financial assistance.

Pharmacist is required to provide a copy of and review the accompanying GRANIX Patient Savings Program Terms and Conditions with the patient prior to redeeming this offer.

GRANIX (tbo-filgrastim) Injection Patient Savings Program.

Terms and Conditions:

  • Subject to program limitations and terms and conditions, the GRANIX Patient Savings program is available to patients who have a valid GRANIX prescription and who have commercial insurance coverage for GRANIX. No substitutions permitted. Patients with commercial insurance coverage that does not provide formulary coverage for GRANIX are NOT eligible for the Patient Savings program. The Patient Savings program does not cover GRANIX dispensed or administered under commercial insurance as adjudicated under a medical plan.
  • Patients enrolled in any state or federally funded healthcare program, including but not limited to, Medicare, Medigap, Medicaid, VA, DOD, TRICARE, Puerto Rico Government Health Insurance Plan, and Medicare-eligible patients enrolled in an employer-sponsored health plan or prescription drug benefit program for retirees, are NOT eligible for the Patient Savings program.
  • Uninsured and cash-paying patients are NOT eligible for the Patient Savings program.
  • Eligible Patients may pay as little as $0 on each fill. Annual benefit limits per individual apply and out of pocket expenses may vary. Patients are responsible for all amounts that exceed these Patient Savings program benefit limits. If the prior authorization is approved by the commercial insurer, then the patient remains eligible for the Patient Savings program. If the prior authorization is denied by the commercial insurer, then the patient is no longer eligible for the Patient Savings program and may not receive any additional Patient Savings program benefits.   • Teva has the right to reduce or eliminate patient benefit amounts, based on factors determined solely by Teva, including depending on the terms of a patient’s prescription drug plan and to ensure all program funds are used for the benefit of the patient.
  • Data related to a patient’s receipt of Patient Savings program benefits may be collected, analyzed, and shared with Cephalon, Inc., a wholly-owned subsidiary of Teva Pharmaceutical Industries Ltd. and its affiliates (“Teva”), for market research and other purposes (including with the patient’s treating physician towards helping to verify or coordinate insurance coverage or otherwise obtain payment for the patient’s treatment with GRANIX) related to assessing the Patient Savings program. Data shared with Teva will be aggregated and de-identified, meaning it will not identify patients.
  • The Patient Savings program is restricted to residents of the United States and United States territories. Patients residing in or receiving treatment in certain states may not be eligible.
  • The Patient Savings program is intended for the benefit of patients, not their insurance plans or other third parties. Patients whose commercial insurance plans do not apply Patient Savings program payments to satisfy patient out-of-pocket cost sharing amounts may not be eligible for the Patient Savings program. Similarly, patients whose commercial insurance plans require use of the Patient Savings program as a condition of the plan waiving some or all of otherwise applicable patient out-of-pocket cost sharing amounts may not be eligible for the Patient Savings program or have a reduced annual maximum program benefit.   • The Patient Savings program is not health insurance. Patients may not seek reimbursement for the value received from the Patient Savings program from any third-party payers, including a flexible spending account or healthcare savings account. Participating in this program means that you are ensuring you comply with any required disclosure regarding your participation in the Patient Savings program of your insurance carrier or pharmacy benefit manager.
  • The Patient Savings program is void if copied, transferred, purchased, altered or traded, and where prohibited and restricted by law. The Patient Savings program is not available to residents of Massachusetts. The Patient Savings program is not transferable. No substitutions are permitted. The Patient Savings program may not be sold, purchased, traded, or counterfeited. Void if reproduced. The Patient Savings program benefit cannot be combined with any other financial assistance program, free trial, discount, prescription savings card, or other o er. Teva reserves the right to make eligibility determinations, to set Program benefit maximums, to monitor participation, and to change, rescind, revoke, or discontinue this Patient Savings program at any time without notice.   • These Terms and Conditions are valid for GRANIX dispensed between 10/1/2022 and 12/31/2023. Expiration Date: 12/31/2023.

  • By redeeming this Patient Savings program, you are certifying that you understand and agree to comply with the Terms and Conditions above.
  • When you apply this offer, you are certifying that you have not submitted and will not submit a claim for reimbursement under any federal, state, or other governmental program for this prescription.

Approved Use

GRANIX is a prescription medicine:

  • used in people with certain types of cancer (non-myeloid malignancies), who are receiving chemotherapy that affects the bone marrow
  • given to help decrease the length of time that the number of certain white blood cells (neutrophils) are very low (severe neutropenia). Neutrophils are white blood cells that are important in fighting bacterial infection
    It is not known if GRANIX is safe and effective in children younger than 1 month of age.

Important Safety Information

Do not take GRANIX if you have had a serious allergic reaction to filgrastim products or pegfligrastim products.

Before you receive GRANIX, tell your healthcare provider about all of your medical conditions, including if you:

  • have a sickle cell disorder
  • have kidney problems
  • plan to have bone scans or tests
  • are pregnant or plan to become pregnant. It is not known if GRANIX will harm your unborn baby. You should not become pregnant during treatment with GRANIX
  • are breastfeeding or plan to breastfeed. It is not known if GRANIX passes into your breast milk

Tell your healthcare provider about all the medicines you take, including prescription and over-the-counter medicines, vitamins, and herbal supplements.

What are the possible side effects of GRANIX?

GRANIX can cause serious side effects, including:

  • Spleen rupture. Your spleen may become enlarged and can rupture. A ruptured spleen can cause death. Call your healthcare provider right away if you have pain in your left upper stomach (abdomen)-area or your left shoulder during treatment with GRANIX.
  • A serious lung problem called acute respiratory distress syndrome (ARDS). Call your healthcare provider or get emergency medical help right away if you have shortness of breath with or without fever, trouble breathing, or a fast rate of breathing.
  • Serious allergic reactions. GRANIX can cause serious allergic reactions. These reactions can cause a rash over your whole body, shortness of breath, wheezing, dizziness, swelling around your mouth or eyes, fast heart rate, and sweating. If you have any of these symptoms, stop using GRANIX and call your healthcare provider or get emergency help right away.
  • Sickle cell crisis. You may have a serious sickle cell crisis, which could lead to death, if you have a sickle cell disorder and use GRANIX. Call your healthcare provider right away if you have symptoms of sickle cell crisis such as pain or difficulty breathing.
  • Kidney injury (glomerulonephritis). GRANIX can cause kidney injury. Call your healthcare provider right away if you develop any of the following symptoms:
    • swelling of your face or ankles
    • blood in your urine or dark colored urine
    • you urinate less than usual
  • Capillary Leak Syndrome. GRANIX can cause fluid to leak from blood vessels into your body’s tissues. This condition is called “Capillary Leak Syndrome” (CLS). CLS can quickly cause you to have symptoms that may become life-threatening. Get emergency medical help right away if you develop any of the following symptoms:
    • swelling or puffiness and are urinating less than usual
    • trouble breathing
    • swelling of your stomach-area (abdomen) and feeling of fullness
    • dizziness or feeling faint
    • a general feeling of tiredness
  • Inflammation of the aorta (aortitis). Inflammation of the aorta (the large blood vessel which transports blood from the heart to the body) has been reported in patients who received another filgrastim product. Symptoms may include fever, abdominal pain, feeling tired, and back pain. Call your healthcare provider if you experience these symptoms.
  • Your healthcare provider may decrease your dose, temporarily stop, or permanently stop treatment with GRANIX if you have certain side effects.
  • The most common side effect of GRANIX is bone pain.
  • Other possible side effects of other filgrastim products that may also happen with GRANIX may include muscle aches (myalgia), headache, weakness, diarrhea, fatigue, vomiting, inflamed blood vessels in the skin (cutaneous vasculitis), low platelet count (thrombocytopenia), and Sweet’s syndrome (a rare skin condition that mainly includes fever and painful skin lesions that appear mostly on the arms, neck, head and trunk).
